ACCESS TO YOUR HEALTH RECORDS: Clients have a right to request a copy of their medical records 

HOW TO SUBMIT A REQUEST: You can request your records by emailing the office at alyssacook@alyssacooklcsw.sprucecare.com or by sending a message through the Patient Portal

HOW RECORDS ARE DELIVERED: To protect privacy and comply with HIPAA regulations, records are delivered electronically via the Patient Portal 

FEES: An administrative fee of $75 applies for a records request to cover preparation, retrieval, and delivery of the documents 

TIMELINE: Records are released with in 30 days of receiving the written request and all required consents are obtained
